Our team of professionals from the fields of aging, adult protective services, civil law, mental health, social work, law enforcement, and others meets monthly to review complex abuse cases and to learn about new service resources and innovative intervention techniques.
Case Presentation
One or two cases are reviewed each meeting. To present a case or to observe a meeting (professionals only), please contact the Consortium for Elder Abuse Prevention at Institute on Aging at: (415) 750-4180, x222.
MDT Presentation Guidelines
- Before placing a case on the calendar for review, please contact the other professionals (if any) who have been working with your client (e.g., APS worker, Meals on Wheels social worker, medical personnel) and arrange for them to present with you. Please designate a lead person who will prepare the written synopsis (doesn't have to be extensive).
- Identifying information: Do NOT use the name of the client or other family members, however, please identify the age, gender, ethnicity, marital status, living situation, mental and cognitive status, functional abilities, and medical history, if relevant.
- Presenting problem: Referral source, safety issues, risk factors, "red flag" issues, financial assessment, support networks.
- Case management treatment plan: What has already been done (i.e., interventions, obstacles). What are your goals?
- What help do you want and/or need from the team?
