How It Works
Senior Dictionary
Program of All-Inclusive Care for the Elderly (PACE)
PACE is a model for community-based managed health care developed
by On Lok. PACE enables seniors with chronic health conditions to
receive care in the community while continuing to live at home.
On Lok Lifeways is a certified PACE Program (Program of All-Inclusive Care for the Elderly). This unique HMO-style health plan was pioneered by On Lok in the 1970s as an alternative to nursing home care, and became the model for a national program. As the name conveys, Lifeways provides “all-inclusive” care for adults 55 or older, enabling participants to continue living at home for as long as possible.
Senior Health Plan
The health plan features full medical care (both acute and long-term), prescription drugs, adult day care, personal care, physical therapy, transportation, and home care. A team of doctors, nurses, and other health professionals assess participants' needs, develop care plans, and deliver all services which are integrated into a complete health care plan.
On Lok Lifeways + IOA
IOA is a contracted provider of On Lok services and manages two Lifeways sites, the Geary and Fillmore Locations.
On Lok's Care Teams work together to provide personalized care to each member and include physicians, nurses, social workers, rehabilitation therapists, dietitians, home care aides, and drivers. Members use On Lok Lifeways physicians who are experts in the care of older adults.

Eligibility
On Lok Lifeways is a possible option for those:
- 55 years old or older and living in San Francisco
- Experiencing ongoing medical problems, memory loss, and requiring daily help with bathing, walking, eating or dressing; and
- who are able to remain in the community with supportive services without jeopardizing their health and safety

| Notes | On Lok Lifeways has no premium, co-payment, or deductible for people who are Medicare AND Medi-Cal beneficiaries AND recipients of Supplemental Security Income (SSI) benefits. Fixed monthly fees may be available for Medicare-only beneficiaries. For people who do not have Medicare, you will be assisted in determining what, if any, monthly fee is required to join. |

On Lok's model of health care began in the early 1970s. San Francisco's
Chinatown-North Beach community recognized that families of immigrants
from Italy, China and the Philippines needed long term care services.
Community leaders formed a nonprofit corporation, On Lok Senior
Health Services, to develop a community based provision of care.
On Lok is Cantonese for "peaceful, happy abode."
In 1996 IOA developed formal affiliations with On Lok to establish
Senior Health centers in the Western Addition and Richmond Districts
of San Francisco, offering a Program of All Inclusive Care for the
Elderly (PACE) provided by IOA.
