IOA’s Assessment Service specializes in examining, diagnosing and treating cognitive, or memory, function. Whether caused by disease or injury, memory loss is confusing and alarming. IOA's Assessment Service offers a thorough individual assessment incorporating expertise from a wide range of care disciplines.

Multi-Disciplinary Care

Memory loss can be the result of a several health and lifestyle issues. In order to diagnose and treat memory loss, the Assessment Team must consider an individual's unique situation and needs from multiple perspectives. To incorporate medical, psychological and social perspectives, specialists in medicine, care management, psychology, neuropsychology, and occupational and physical therapy work together to diagnose and evaluate the needs of each client, and to make specific recommendations for optimal care.

Comprehensive assessment

Comprehensive assessment of a client's strengths, vulnerabilities and needs is conducted in a caring and respectful manner. A client’s support structure is also considered.

Care Recommendations

Specific recommendations for care are made based on a thorough understanding of the client's medical, psychological, cognitive, functional and spiritual needs. When appropriate, key people involved in the client’s life are brought into discussions about future care.

House calls

Assessments are available in the client’s home, at our clinic, or at another location convenient to the client.

Short wait lists

Clients are seen within two weeks of initial contact. Assessments are completed within two weeks of our last visit with the client. Urgent requests are also accommodated whenever possible.

 

Goals of the Assessment Service
  • To provide a comprehensive assessment of each client’s strengths, vulnerabilities and needs in a caring and respectful manner.
  • To make specific recommendations for care, based on a thorough understanding of the client’s medical, psychological, cognitive, functional and spiritual needs.
Who Requests an Assessment

Request for assessments are generated by service providers (physicians, visiting nurses, social workers, Adult Protective Services), attorneys, family members, and by clients themselves.

What does Assessment include?

Every client is unique, but some common problems that we evaluate include:

  • memory loss
  • falls
  • home safety
  • medication management
  • caregiver needs
  • family stress
  • functional abilities and problems with independent living
  • capacity to make medical or legal decisions
  • Information and referral to resources, both within IOA and outside, are also provided.

Who is eligible

All adults with reduced congnitive capacity (memory loss or dementia) are eligible for IOA's Geriatric Assessment Services. Our primary expertise is in assessment for cognitive capacity in cases of dementia. However, the Assessment Service can also work with younger adults to assess cogntive deficits related to brain injury, substance use, or other issues.
