Service Overview
IOA’s Assessment Service provides a thorough, multidisciplinary assessment, carefully considering each patient’s interests, needs, and challenges from not only the medical, but also the psychological and social perspectives. Specialists in medicine, care management, psychology, neuropsychology, and occupational and physical therapy work together to diagnose and evaluate the needs of each client, and make specific recommendations for optimal care.
What We Offer
- Comprehensive assessment of a client's strengths, vulnerabilities and needs, conducted in a caring and respectful manner. A client’s support structure is also considered.
- Specific recommendations for care, based on a thorough understanding of the client's medical, psychological, cognitive, functional and spiritual needs. When appropriate, key people involved in the client’s life are brought into discussions about future care.
- House calls—Assessments are available in the client’s home, at our clinic, or at another location convenient to the client.
- Short wait lists—Clients are seen within two weeks of initial contact. Assessments are completed within two weeks of our last visit with the client. Urgent requests are also accommodated whenever possible.
- Information and referral to resources, both within IOA and outside, are also provided.
Every client is different, but some common problems that we evaluate include:
- memory loss
- falls
- home safety
- medication management
- caregiver needs
- family stress
- functional abilities and problems with independent living
- capacity to make medical or legal decisions
Eligibility & Fees
Medicare and other insurances are billed when possible. We can accept some managed care insurance, and for uninsured clients we offer discounts and payment plans.
