Medi-Cal Health Homes Program (HHP) of CA Dept of Health Care Services

On Tuesday, March 10th, Institute on Aging will offer a 90-minute workshop, covering the new, extra Medi-Cal services available through the California Health Homes Program (HHP).  The HHP is a program of the California Department of Health Care Services.

The HHP offers extra services at no cost to Medi-Cal members with certain chronic physical and/or mental health issues, and who have high health care needs or no place to live.

The workshop will explain how Care teams work together to plan and manage a Medi-Cal member’s care, connecting a member to community and social services (with an assigned care coordinator as the main point of contact). Among the services which will be available to the Medi-Cal member will be food, housing, medical providers, transportation to and from medical appointments, and follow-up services (after members leave a hospital or nursing facility).

By the end of this program, attendees will be able to:

  • Describe the roles of the two primary Care team members
  • Name the three requirements which all potential Medi-Cal members must meet, in order to access HHP services
  • Outline and implement the 6 HHP Core Services

YOUR INSTRUCTOR:

Chris Flannery is an Outreach Specialist with Harbage Consulting, where he works on projects related to educational outreach, stakeholder engagement and the implementation of public health programs in California.
Currently, Chris works with the California Department of Health Care Services (DHCS) to implement the Health Homes Program (HHP), which is a new health care program that gives qualifying Medi-Cal beneficiaries additional support. Before joining the Harbage Team, Chris was a project manager and medical scribe for physicians representing varied medical disciplines and hospitals throughout Southern California. As a program manager, he was responsible for interviewing, hiring, and training personnel for several medical transcription
programs he built from the ground up. Additionally, his experience working formerly as an emergency paramedic and fire fighter supplements his medical expertise and informs his decision-making when assisting his colleagues
in implementing public health programs. Chris holds a Bachelor of Arts in Musicology from the University of California at Davis and an Associate of Science in Emergency Medical Services – Paramedic
from Mt. San Antonio College in Walnut, CA.

DateTuesday, March 10, 2020, 12:30 p.m.- 2 p.m. Doors to the training will open at 11:45 a.m. The program will begin on time, so please be prompt.

Directions and Parking: Limited street parking around the building, in mostly 2-hour increments. Public transportation is nearby, with the Geary bus running right in front of the building.

If you seek 1.5 professional Continuing Education Units, please note this in your reservation. There will be no charge for this.

Also note that BBS no longer delivers CEs. CAMFT is an approved BBS provider, and offers CEs to  MFTs and LCSW, LPCCs and LEPs.

Introductory Level: No knowledge of the specific content area is required—you need not have in-depth knowledge or skills. The activity will provide information in accordance with basic knowledge of the topic.

All Continuing Education certificates will be delivered digitally, within 5 working days of the event. An evaluation-test will be provided to all CE registrants, who must then attend the entire program and turn in their evaluation at the end of the program in order to qualify for a CE certificate.

American Psychological Association (APA): Institute on Aging is approved by the American Psychological Association to sponsor continuing education for psychologists. Institute on Aging maintains responsibility for this program and its content.

Provider approved by the California Board of Registered Nursing (BRN), Provider # CEP13610, for 1.5 Contact Hours.

CAMFT: Institute on Aging is approved by the California Association of Marriage and Family Therapists to sponsor continuing education for this program. IOA maintains responsibility for the course and its content. Course meets the qualifications for 1.5 hours of continuing education credit for LMFTs, LCSWs, LPCCs, and/or LEPs. CAMFT provider number is #135377.

State Bar of California (MCLE) : Course meets the qualifications for 1.5 hours of continuing education credit for Attorneys (Provider #13264).
