IOA Case Management

IOA Community Case Management helps older adults, adults living with disabilities, or their caregivers coordinate all the services they may need in order to continue to live in the place they call home.

IOA Community Case Management is a partnership between the City of San Francisco’s Department of Disability and Aging Services (DAS) and several case management providers throughout San Francisco County. IOA is one of many non-profit providers of this service.

 

This program serving the community is funded by the City of San Franciscoโ€™s Department of Disability and Aging Services.

Program Staffing

  • Care Managers
  • Clinical Supervisor

Eligibility Requirements

  • Resident of San Francisco
  • Age 60+ or 18-59 and living with disabilities
  • At risk of institutionalization
  • Impairment in one or more ADLs
  • Impairment in two or more IADLs
  • Unable to manage his/her own affairs due to emotional and/or cognitive impairment
  • Impaired by virtue of a significant event or circumstance that has occurred within the past 12 months
