Enhanced Care Management (ECM) is a free Medi-Cal benefit that gives you a single lead care manager who coordinates your doctors, specialists, mental health care, medications, housing, food, transportation, and benefits. Your care manager meets you where you are comfortable: at home, at a clinic, in the hospital, or out in the community.
You do not pay anything. Joining is your choice, and you can stop at any time.
Enhanced Care Management covers seven core services. In plain terms, here is what that means day to day.
Outreach and engagement
We come find you, including if you are unhoused or have lost touch with your doctors.
A full assessment and a care plan
We look at your medical, mental health, dental, and everyday needs, and you help write the plan.
Coordinating your care
One person talks to your doctors, specialists, pharmacist, and county programs on your behalf.
Health coaching
Practical help with medications, chronic conditions, and the routines that keep you out of the hospital.
Support through hospital stays and moves
We plan with you before you leave the hospital, and we are still there once you are home.
Help for your family and caregivers
The people caring for you get education and hands-on support too.
Connections to community services
Housing, food, transportation, and benefits, because recovery depends on those as much as on medicine.

Nothing. Enhanced Care Management is a covered Medi-Cal benefit provided at no cost to eligible members. There are no copays and no bills.
Call Institute on Aging at 415.750.4111. You can also ask your Medi-Cal health plan, your doctor, or a hospital care manager to refer you. We confirm your eligibility with your health plan, then a care manager reaches out to get started.
No. Enhanced Care Management is added on top of the care you already have. You keep your doctors and your health plan. Your care manager works with them, not instead of them.
It depends on what is going on in your life. Some people need weekly contact during a difficult stretch, others need less. Care happens in person wherever you are comfortable, including at home, and your care manager is your single point of contact between visits.
Yes, if that is what you want. Support for family members and caregivers is one of the seven core Enhanced Care Management services. Your care manager can bring them into planning and connect them to education and support of their own.
Participation is completely voluntary. You can opt in or out at any time, and it does not affect your Medi-Cal coverage or any other benefit.
Institute on Aging serves members in the language they prefer, including Spanish, Chinese, and Russian, with interpretation available for other languages. Tell us what you need when you call.
Enhanced Care Management is the person who coordinates everything. Community Supports are specific services your health plan can approve, such as help finding housing, meals designed for your health condition, home modifications, or a safe place to recover after a hospital stay. Many people get both, and your care manager is usually the one who arranges the Community Supports.
