Proof of concept. This is the partner landing page template we discussed, using Sonoma County as the example. Every element here is meant to be driven from a back end: partner name, logo, intro paragraph, the services available to that partner’s population, and the referral destination. The same template would produce a page for San Francisco Health Plan, Zuckerberg San Francisco General, Sutter Health, or any other partner at a short URL such as ioaging.org/sonoma or ioaging.org/sfhp.

This is the piece that needs a decision. In the version, this block holds a partner-specific referral form that submits into HubSpot, so IOA can see that a referral came from the Sonoma Hub without the referrer having to say so. Routing would be handled by HubSpot rather than emailing the care team directly, which keeps the data capture inside a HIPAA-compliant system.
