Together, we celebrated the connections Institute on Aging creates, and honored Dick Levy, whose leadership has helped shape health care and aging services in our community for decades. His commitment to innovation, excellence, and compassionate care has made a lasting impact at Institute on Aging and across the region.
Through Dick’s early support of Institute on Aging’s Acute Care Transition (ACT) pilot, care managers are now embedded in San Francisco emergency departments, helping connect older adults to critical support and strengthening the link between clinical care and community services.
