If you or someone you know is facing issues related to aging or disability

Call Institute on Aging Connect if any of the following apply to you or someone you know . . .

  • has dementia, frailty, or a disability but wants to avoid a nursing home.
  • spends too much time alone.
  • makes too many trips to the ER.
  • may be a victim, or at risk, of abuse—physical, psychological or financial.
  • can no longer manage routine finances, medications or daily tasks.
  • has a low income and health problems that are not being addressed.
  • still wants to be active but has no outlet.
  • has reason to grieve.

If you are caring for a loved one with dementia, frailty or a disability

Call Institute on Aging Connect if you . . .

  • are exhausted from the workload and stress.
  • work outside the home and don’t want to leave your loved one alone.
  • are experiencing family disagreements about what to do.
  • are facing difficult decisions alone.
  • need help or information but don’t know who to ask.

If you are a professional working with older or disabled adults

  • wish to make a professional referral for Institute on Aging services.
