Institute on Aging has served San Francisco since 1985, and our main office is on Geary Boulevard. We deliver CalAIM Enhanced Care Management across the city, including bedside enrollment at San Francisco hospitals through our Acute Care Transitions program.
You may be a good fit if you are 18 or older, have Medi-Cal through a managed care plan, and have ongoing health or social needs that are hard to manage alone.
San Francisco Health Plan is also our founding partner in the Acute Care Transitions program, which enrolls eligible members into Enhanced Care Management while they are still in the hospital.

IOA care managers are embedded in San Francisco acute care settings and can enroll an eligible patient before discharge. If you are a discharge planner, care manager, or social worker, call 415.750.4111 as early in the stay as you can.
