IOA Community Case Management helps older adults, adults living with disabilities, or their caregivers coordinate all the services they may need in order to continue to live in the place they call home.
IOA Community Case Management is a partnership between the City of San Francisco’s Department of Disability and Aging Services (DAS) and several case management providers throughout San Francisco County. IOA is one of many non-profit providers of this service.
This program serving the community is funded by the City of San Franciscoβs Department of Disability and Aging Services.
We recently had the pleasure of speaking at the Aging Well Summit hosted by @Kaiser San Leandro. It was a fantastic turnout with attendees all dedicated to the cause of aging in place. We`re proud to connect and share insights with other organizations committed to this important work. Together, we`re making a difference! #AgingWell #CommunityEngagement #InstituteonAging

Today on #HolocaustRemembranceDay, we at IOA honor the memory of the lives lost. At IOA, we recognize that remembrance is not just about looking backβit is a call to action. Together, we can honor the memories of those who suffered, support the resilience of survivors, and commit to building a world free of hate and prejudice.
