You may be a good fit if you are 18 or older, have Medi-Cal through a managed care plan, and have ongoing health or social needs that are hard to manage alone.
Sonoma County is a County Organized Health System county, which means Medi-Cal there is administered through a single local plan. Partnership HealthPlan of California is that plan, and it is our Enhanced Care Management partner in Sonoma County.
Some Sonoma County residents who were already Kaiser Permanente members have Medi-Cal through Kaiser instead. If you are not sure which plan you have, call us and we will check for you.
