This program launched in 2014 as the Community Care Setting Pilot, then transitioned into CalAIM San Mateo in 2022. MediCal delivery changed to an Enhanced Care Management model with community supports in order to prioritize whole person care and promote healthy living.

  • Understanding prescriptions and medications
  • Follow-up services after you leave the hospital

  • Connecting to other community programs and services
  • Housing navigation
  • Housing deposits
  • Coordinating transition out of a skilled nursing facility into a home-like environment
  • Skilled nursing admissions prevention
  • Securing meals to meet nutrition goals

Eligibility Requirements

  • Be a Health Plan of San Mateo member
  • Have multiple chronic or mental health conditions
  • Have visited an ER or hospital multiple times in the last 6 months
  • Willing to move to a home from a nursing facility
  • Are in need of a safe place to live

Meet Pete, a proud Californian who faced significant challenges after suffering heart attacks in 2008. He ended up in a Skilled Nursing Facility and lost his subsidized apartment, leading to 12 years of isolation and depression. But with the help of the Institute on Aging, Pete's life took a remarkable turn.

We specialize in transitioning individuals back into the community, and Pete's story is a shining example. With our support, he moved into a beautiful Senior Housing apartment in January. Today, he's thriving, decorating his apartment, and enjoying a newfound sense of independence. 🏡
