
Home Safe is a program intended to support the safety and housing stability of individuals within Adult Protective Services (APS), which supports victims of abuse and/or neglect who are either adults age 60+ or adults ages 18-59 who are living with disabilities.
Home Safe provides intensive case management and other supportive services to individuals experiencing or at imminent risk of homelessness due to elder or dependent adult abuse, neglect, self-neglect or financial exploitation.
We are partnering with Residential Care Facilities for the Elderly (RCFE) across the Bay Area to find appropriate placements for vulnerable individuals needing RCFE levels of support.
By contracting with Institute on Aging, you may have access to a larger pool of client referrals, consistent reimbursement for services, and increased community exposure. We can also guide you through the process of connecting with additional support from state-funded insurance and waiver programs.
